Our Enrollment Requirements
The forms below, when returned with the non-refundable $50 registration fee, will assure your child's place in the Milwaukie Preschool. Until the fee is paid and the papers turned in, an enrollment is not held for you. It is to your advantage to return the enclosed forms as soon as possible.
Requirements for enrollment: Child must be of age on or before September 1st of the current school year and must be fully potty trained.

Please read explanation of membership requirements carefully. Sign and return. You will find an additional copy of this agreement in your handbook for future reference
Church Waiver Form (PDF)
This is a legal document required by the church from whom we lease our classroom space.
Please complete and return all forms to the Registrar as soon as possible with the non-refundable $50.00 registration fee, payable to Milwaukie Preschool. The first month's school tuition is due the first day of school and the last month tuition is paid half in October and half in November.
